Primary Health Care Services

Primary Health Care (PHC) Services represent the first level of contact between individuals, families, and the health system. These services provide essential, accessible, and affordable health care focused on disease prevention, health promotion, early diagnosis, treatment, and continuity of care, with an emphasis on community-based and people-centered approaches.

Scope of Services

  1. Preventive Health Services
    Delivery of immunization programs, maternal and child health services, nutrition support, family planning, and health education aimed at preventing diseases and promoting healthy lifestyles.

  2. Curative and Clinical Care
    Diagnosis and treatment of common illnesses and injuries, including infectious diseases, non-communicable diseases, and minor emergencies, at health centers and community clinics.

  3. Maternal, Newborn, and Child Health (MNCH)
    Antenatal care, skilled delivery referrals, postnatal care, newborn care, growth monitoring, and management of childhood illnesses.

  4. Reproductive and Adolescent Health Services
    Family planning counseling, reproductive health services, adolescent-friendly health care, and prevention and management of sexually transmitted infections.

  5. Community Health and Outreach Services
    Community-based health promotion, disease surveillance, home visits, outreach clinics, and engagement of community health workers to extend services to underserved populations.

  6. Management of Chronic and Non-Communicable Diseases
    Screening, treatment, and follow-up care for conditions such as diabetes, hypertension, asthma, and mental health conditions at the primary care level.

  7. Referral and Continuity of Care
    Identification and referral of patients requiring specialized or secondary care, with follow-up to ensure continuity and coordination across the health system.

Service Standards and Quality Assurance

  • Delivery of services in accordance with national health policies and WHO Primary Health Care guidelines

  • Use of qualified and licensed health professionals

  • Availability of essential medicines, medical supplies, and basic diagnostic services

  • Application of infection prevention and control (IPC) measures

  • Routine health data collection, reporting, and monitoring
